Similar experience is not uncommon in several countries, Australia being one of them, even though the causes were somewhat different (www.ruralhealth.org.au/hh/htm) Five main reasons have been identified as the cause of this situation, namely:  Focus on the construction of facilities rather than the provision of services.  Over-sophisticated training, which took place largely in hospitals.  Poor and un-equitable deployment of health staff  A “top-down” health care delivery system.  Unbalanced health budget allocations. In the light of these problems concerning the existing health care system, South Africa is now evolving more “effective” ways of improving health in the communities.
