Item The Prevalence of Arrhythmias in Acute ST-Segment Elevation Myocardial Infarction(University of the Witwatersrand, Johannesburg, 2024) Setshego, Mothusi Koketso; Tsabedze, Nqoba; Patel,, AnupaSouth Africa has a rising prevalence of ischaemic heart diseases. Patients with acute myocardial infarction who succumb to sudden cardiac death tend to have fatal arrhythmias. This study aims to determine the prevalence of arrhythmias in patients with acute ST-segment elevation myocardial infarction (STEMI). We retrospectively reviewed medical records of patients with STEMI at a tertiary academic hospital for a 5 year period between 1 January 2015 and 31 December 2019. The study population comprised 259 patients with a mean age of 56.0 ±10.8 years. Dyslipidaemia was the most common co-morbidity, found in 169 (65.2%) patients. There were 125 (48.3%), 123 (47.5%) and 11 (4.2%) patients with inferior, antero-septal and lateral myocardial infarction, respectively. The most common arrhythmia was a sinus tachycardia, found in 80 (12.1%) patients, followed by a sinus bradycardia in 37 (5.6%). Thirty-seven of the 80 (70.1%) patients with a sinus tachycardia had ST-elevation in the anterior leads, and 19(51.3%) with a sinus bradycardia had ST-elevation in the inferior leads (p<0.009). Among the 143 patients with left anterior descending artery stenosis, 72 (50.3%) had a sinus tachycardia, while 19 (13.3%) had a sinus bradycardia (p<0.001). The most common arrhythmias in STEMI patients were a sinus tachycardia and bradycardia, accounting for arrhythmias in 36 and 27% of all patients, respectively. Conclusion:The most common arrhythmias in ST-elevation MI patients were a sinus tachycardia andbradycardia, accounting for arrhythmias in 36% and 27% of all patients, respectively. Future prospective and multicentre studies are required in low-and-middle-income countries, which will determine the true burden of arrhythmia in acute STEMI.
